We have been a provider of at-home Therapy services since 2016, our services include Physical, Occupational, Speech, and wellness programs for seniors. The growing team provides specialized therapy services at the comfort of the patient's home. Denver Physical Therapy at Home is focused to establish a good relationship with the clients and community in order to best serve the geriatric population. NO OASIS to complete as we are a mobile outpatient clinic. The therapist has a flexible schedule and independently manages their own time.

Job Overview

• Perform a detailed evaluation and assessment on all clients
• Ability to communicate with the patient, physician, team members, and family
• Provides completed and timely documentation in the EMR system
• Develop individualized plans of care for clients
• Determine treatment goals, strategies, and outcome plans
• Records clients progress and modify treatment as needed
