ABOUT METROCARE:
Metrocare is the largest provider of mental health services in Dallas County, serving more than 57,000 adults and children annually. For more than fifty (50) years, Metrocare has provided an array of services available to people with mental and developmental disabilities. In addition to behavioral health services, Metrocare provides primary care centers for adults and children, accessible pharmacies, housing and supportive social services. Through the Altshuler Center for Education & Research we are advancing mental health beyond Dallas County.

GENERAL DESCRIPTION:

The mission of Metrocare Services is to serve our neighbors with developmental or mental health challenges by helping them find lives that are meaningful and satisfying. We are an agency committed to quality gender-responsive, trauma-informed care to individuals experiencing serious mental illness, development disabilities, and co-occurring disorders. Metrocare programs focus on the issues that matter most in the lives of the children, families, and adults we serve.

The Registered Nurse provides nursing assessment and supports in a home visit setting to families with newborns in the area(s) assigned by the Family Connects Nurse Lead. The nurse supports and assesses families in 12 domains of physical and psychosocial well-being and facilitates referrals and connections to community agencies to support families. The nurse uses the nursing process in nursing assessments, planning, implementation/intervention, education, evaluation, and consultation. The nurse is a member of the interdisciplinary treatment team and works within the team environment to schedule, visit and support families using the evidence-based Family Connects home visiting protocol.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Follows the Family Connects scheduling protocol, including productivity requirements as outlined by supervisor.
  • Keeps records of client activities and documents assessments and visits in real time via in-home use of computerized record system. All nursing progress notes conform to Metrocare Services standards as well as Medicaid standards and must be completed within 24 hours.
  • Provides health and physical assessment of mother and baby in the home to determine well-being. Assesses family needs in 12 domains of physical and psychosocial well-being, including depression, substance abuse, and domestic violence. Exercises clinical skills in appraising situations and making decisions.
  • Establishes a trusting relationship with families during home visits and engage families to establish support plans with goals, objectives and activities that address family strengths and needs.
  • Applies working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assists in creating a health plan for entire family.
  • Works with Family Connects team by attending weekly team meetings, clinical supervision, staff meetings, and other meetings as required. Supports and encourages development of other staff within Family Connects.
  • Maintains necessary supplies for implementation of agency policies and procedures on infection control, according to the Infection Control Manual. Carries out measures to prevent exposure to and transmission of pathogens in accordance with agency standards on infection control.
  • Performs other duties as assigned.

  • Pass all DMS & Family Connects required training.
  • Ability to work a flexible schedule.
  • Ability to make basic independent decisions with good judgment, attention to detail, and crisis interventions.
  • Active listening skills; cultural sensitivity
  • Understanding of medical terminology and its appropriate use.

QUALIFICATIONS

EDUCATION AND EXPERIENCE:

  • BSN, or A. DN from an accredited school of nursing
  • Must have 2-3 years of experience, preferably in community setting.

MATHEMATICAL SKILLS:

  • Basic math skills required.
  • Ability to work with reports and numbers.
  • Ability to calculate moderately complex figures and amounts to accurately report activities and budgets.

REASONING ABILITY:

  • Ability to apply common sense understanding to carry out simple one or two-step instructions.
  • Ability to deal with standardized situations with only occasional or no variables.

COMPUTER SKILLS:

  • Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
  • Ability to utilize Internet for resources.

CERTIFICATIONS, LICENSES, REGISTRATIONS:

  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business. Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.

PHYSICAL DEMANDS & WORK ENVIRONMENT

PHYSICAL DEMANDS:

While performing the duties of this job, the incumbent is regularly required to talk and hear, use hands and fingers to operate a computer and telephone. Due to the multi-site responsibilities of this position the incumbent must be able to carry equipment and supplies (up to 15 pounds). The position requires standing and/or walking in excess of 7 hours a day.

TRAVEL:

  • In county travel is required to meet the needs of the newborns and families assigned to caseload.

Positions that are community-based, in whole or part, require the incumbent to travel between various worksites within his/her workday/workweek. The incumbent is required to have reliable transportation that can facilitate this requirement. The incumbent is further required to meet the criteria for insurability by the Centers risk management facilitator; and produce proof of minimal auto liability coverage when applicable. Failure to meet these terms may result in disciplinary action up to and including termination of employment, contract or other status with Metrocare.
