We provide care co-ordination services to our clients for Chronic Care Management (CCM) / Remote Patient Monitoring (RPM) & Principal Care Management (PCM). We are located in Warrenville, IL and serve clients in all over Chicagoland area.

We are looking for candidates with experience as a receptionist and medical assistant to work with our care providers team. The candidate will be responsible for a variety of administrative and clinical tasks, such as attending to phone calls, patient scheduling, monitoring vital signs, communicating with patient and physician's to the adhere with care plans, and helping patients with prescription refills. The ideal candidate will have previous experience in the medical field, be comfortable working with people in the medical environment and enjoy helping others.

Responsibilities:

• Create and Follow care plans protocols with patients on a regular basis.
• Educate patients about various conditions and treatment options.
• Serve as a health educator for patients.
• Gather and document patient information.
• Monitor patient vital information for analysis.
• Compile data about patients' medical history and condition.
• Communicate with physicians to ensure effective treatment of patient.

Knowledge of CCM (cronic care management) and RPM (remote patient monitoring) programs is a big plus.

Job Types: Full-time, Part-time, Contract

Pay: $19.00 - $20.00 per hour
