Entry Level Supervisor - Individual Plan Coordinator

  • Starting wage is $23.00 per hour
  • Wage scale is $23.00- $27.91 per hour

Role: The Individual Plan Coordinator (IPC) position is similar to a Case Manager. The IPC will coordinate, manage, and oversee the implementation of the Individual Instruction and Support Plans (IISP’s) for all clients on their caseload. Additionally, oversight and assessment of collected data about those plans, providing feedback and training to management and staff teams, along with regular involvement with clients, peers, and stakeholders that also provide support to that client or PSRS.

Schedule: Monday-Friday 9am-5pm, schedule fluctuates depending on program needs, which may include evenings and weekends.

Location: Administrative Office located in Downtown Renton, WA and occasionally visiting client homes. This is not a remote position.

This position requires travel throughout King County- Renton, Kent, Covington, Maple Valley, Federal Way, SeaTac, and Seattle.

  • Employed 2 years or less: 0.56 hours/40 hours worked (approx.3.62 days/year for FT employee)
  • Employed 2- 4 years: 1.5 hours/40 hours worked (approx. 8.12 days/year for FT employee)
  • Employed 5+ years: 1.71 hours/40 hours worked (approx. 11.12 days/year for FT employee)
  • Paid Sick Leave- Accrues immediately, available for use on the 90th day of employment. Accrual rate for all employees is 1 hour per 30 hours worked.
  • 8 paid holidays- floating paid day off for identified holidays.
  • Bereavement pay- paid time off to grieve and heal due to the passing of a family member. List of family members is defined.
  • Agency device- Choice between a device plan allowance with a device stipend or an employer provided device.
  • Sign-on bonus- $250.00 after completing 6 months of employment.
  • Referral bonus- Refer a new employee, bonus received when referred employee completes 6 months of employment.
  • Full time referral (30+ hours/week): $250.00 bonus
  • Part time referral (0-29 hours/week): $125.00 bonus
  • Paid trainings- CPR/1st Aid, Bloodborne Pathogens, Nurse Delegation Core, 75-hour certificate (issued once 5-hour Orientation and Safety, 40-Hour Core Basic, and 30 hours Population Specific is completed), Continuing Education, and any additional PSRS identified trainings.
  • Paid license- Initial application and annual renewal of Nursing Assistant- Registered or Nursing Assistant – Certified.

Responsibilities: (including, but are not limited to):

  • Plan and coordinate IISP meetings with stakeholders.
  • Develop, draft, monitor, and update Individual Instruction and Support Plans (IISPs) for all clients within assigned caseload.
  • Develop, draft, monitor, and update Positive Behavior Support Plans (PBSPs) for all clients requiring a PBSP within assigned caseload.
  • Monitoring and oversight of information collected by the team (Staff and Management) to report back on skill development, suggestions for changes necessary for the plans, and communication with the PSRS team and DDA (Developmental Disability Administration).
  • Collaborate with other IPCs within the agency and work as part of several active teams to develop and communicate information that is pertinent within those teams.
  • Coordinate with our clients, DDA Case Resource Manager (CRM), PSRS stakeholders, and extended service team members to schedule IISP meetings for all clients within assigned caseload.
  • Lead new hire training and ongoing training for current staff for client IISPs, PBSPs, and other plans/documents/systems for all clients within assigned caseload.
  • Provide emergency/after hours on-call availability as needed, and as scheduled/planned (IPCs are not primary on-call – only as needed or requested by Directors).
  • Communicate effectively and collaboratively with clients, co-workers, peers, families/guardians, employers, consultants, and medical providers.

Position Qualifications and Requirements (including, but are not limited to):

  • Bachelor’s degree strongly encouraged along with experience in this field of work. HS diploma / GED is required.
  • PSRS may consider a combination of excellent experience working with individuals with developmental and intellectual disabilities, other related employment, and skills that are commensurate with the responsibilities of the position.
  • Must possess excellent writing skills and competency with technology.
  • Ability to communicate clearly and effectively.
  • Ability to communicate in American Sign Language or willingness to learn.
  • Supervision and Leadership - Ability to collaborate effectively and professionally.
  • Ability to apply good judgment in a highly ambiguous work environment.
  • Demonstrate experience managing shifting priorities, accomplishing multiple tasks effectively, meeting deadlines and consistently providing follow through on routine and emerging responsibilities.
  • Experience working independently and in cooperation with a team.
  • Ability to positively receive constructive criticism, critical feedback, and learn from mistakes.
  • Knowledge of DDA policies and State laws that impact and guide supports for individuals who have developmental and intellectual disabilities.
  • Valid Washington State Driver’s License, auto insurance and a reliable vehicle
  • Clean driving record.
