We have an exciting opportunity for an intake coordinator. It is remote but the successful job candidate must live in MA reasonably near Jamaica Plain. The position performs new patient intake, scheduling Home Health Aides (HHAs), Also, hiring HHAs and homemakers for our Boston Office.

We are located in Jamaica Plain.

This is a remote position that needs to cover the office after hours, including weekends, holidays and other occasions where the office is closed.

Specific hours:

Mon-Fri (6:00pm - 8:00pm)

Sat-Sun (9:00am - 5:00pm)

Holidays + Other office closures (9:00am-5:00pm)

Some training at our Head Quarters in Jamaica Plain will be necessary to begin.

Key Tasks:

Patient Intake

• Receive and coordinate patient referrals for the Boston branch. That is, new home health patient admission.
• Typically ensure all clients are scheduled within two days of the referral
• Track referrals
• Update records and prepare reports for tracking referrals
• Communicate and build relationships with referrers
• Participate in on-call rotation

HHA Performance Management

• Match HHAs with clients based on HHA skills, availability and patient needs
• Creating and communicating caregiver schedules
• Ensure fairness in scheduling hours and minimize overtime
• Initial scheduling of new client plan of care
• Ensuring HHAs perform their jobs

Hiring

• Contact Home Health Aide applicants for the Boston branch
• Conduct Interviews and reference checks
• Gives the list of approved applicants to HR and for orientation
• Reviews and submits the completed hiring package for the new employees to HR.

Job Type: Full-time

Pay: Up to $19.00 per hour
