The nonprofit San Francisco Campus for Jewish Living serves a significant number of patients and long-term care/skilled nursing residents each year in its Jewish Home & Rehab Center. Non-residential care is offered through our short-term and rehabilitation services unit for patients who require specialized interdisciplinary rehabilitation to recover from surgery or an acute illness, and through our acute geriatric psychiatry hospital. With a complement of close to 650 compassionate, experienced clinical and allied health personnel, SFCJL provides older adults with a variety of life-enriching healthcare programs and services in a welcoming, stimulating environment.
SFCJL’s transformed campus includes the new Lynne & Roy M. Frank Residences (assisted living apartments and memory care & support assisted living suites), and Byer Square, a non-residential community of programs, services, and support.

Position Overview:
The Director, Social Services is responsible for the overall organization and coordination of social service delivery to residents, patients, families, as well as supervision of social services staff. Areas of responsibility include discharge planning, clinical supervision, behavioral management programs, program development, professional outreach, translation services and public benefits.
Essential Function:
- Work closely with Administrator, Director of Nursing, department heads, clinical staff and social work team to meet the psychosocial needs of residents patients and families.
- Work collaboratively with Director of Admissions and physicians to coordinate admissions and ensure smooth process.
- Develop and maintain written policies and procedures for social service delivery that are compatible with the overall philosophy and purpose of Jewish Home, as well as compliant to applicable regulations.
- Determine/develop a staffing plan for the social service department that maximizes the effectiveness and efficiency of social service delivery.
- Interpret and ensure compliance with legislative requirements for residents and patient care.
- Set the agenda and lead the weekly social service staff meeting.
- Oversee transfers and coordinates room changes when needed.
- Responsible for timely completions of MDS assessments as they pertain to social services and serve as a resource for social workers in this area.
- Work collaboratively with IDT to implement non-psychological interventions to address resident/patient behavior issues and pain management interventions.
- Partner with administrative staff to facilitate resident services.
- Responsible for administrative tasks and record-keeping to ensure the optimal functioning of the social service department.
- Maintain residents theft and loss logs, oversee management of reimbursement for missing items.
- Participate in Palliative Care committee meeting, staff meetings and educational in-services as required.
- Prepare monthly QAPI process/committee meeting, staff meetings and educational in-services as required.
- Oversee discharge planning process and serve as a resource for discharge planners.
- Coordinate with psychologists and psychiatrists to facilitate psychosocial care
- Provide psychological support to STARS patients as needed
- Participate and facilitate support groups such as grief support
- Function as the Outreach Coordinator to consumer groups, and to the social services' professional community.
- Prepare monthly QI audits and compiles data to present at monthly QI meeting.
- Oversee transitional / discharge planning process and serve as a resource for transitional care coordinators.
- Coordinate with psychologists and psychiatrists to facilitate psychosocial care.
- Provide psychological support to patients and residents as needed.
- Participate in Psychotropics Committee; organize and maintain monthly psychotropics and behavior monitoring review with clinical staff.
- Oversee crisis intervention, mental health 5150 assessments as needed.
- Provide clinical supervision to applicable personnel (e.g. Social Service Director on the Acute Psychiatric Unit).
- Perform other duties as assigned
Qualifications:
- Current/Valid licensure as a Licensed Clinical Social Worker (LCSW) - required
- Two year’s supervisory experience, required
- Two year’s geriatric social work experience in a skilled nursing facility, preferred
- Demonstrated ability to build rapport quickly with peers and customers
- Demonstrated ability to maintain confidentiality
- Demonstrated ability to communicate effectively with patients and their families, and at all levels of the organization
- Demonstrated excellent communication skills including verbal, written and presentation
- Demonstrated proficiency with MS Office, including Outlook, Word, Excel; proficiency with data entry and use of electronic health record software
- Knowledge of State and Federal laws and regulations governing mental health
- Knowledge of mental health community resources
- Ability to be accurate, concise, detail oriented and make independent decisions
- Passionate about quality care and commitment to excellence, able to think outside the box
