Remote Position
Responsibilities
- Provision of remote, short-term case management services to 0-21 aged youth in Hunterdon, Somerset, and Warren counties
- Responsibility for linkage and referral to appropriate community resources for assigned youth and families during 8-week stabilization period following initial crisis intervention, providing education on crisis prevention strategies, support in transitions of life/placement for children
- Ability to effectively liaison and interact with community providers required
Requirements
- Minimum of Bachelor’s degree in social sciences, social work, or related field
- Experience within Children’s System of Care preferred
