The ALS Association Greater New York Chapter is one of the largest and most active chapters in The ALS Association’s nationwide network of chapters. The ALS Association Greater New York Chapter is a leader in the fight to cure and treat ALS (Amyotrophic Lateral Sclerosis, also known as Lou Gehrig’s Disease) by funding global, cutting edge research, and by providing vital support services to people with Lou Gehrig’s Disease and their families in the Greater New York metropolitan area.

The ALS Association Greater New York Chapter is seeking an experienced licensed social worker to provide comprehensive support services and coordination of multi-disciplinary care services to help improve the quality of life of patients and their families living with ALS in New York City and Western Long Island. ALS is a fatal and neurodegenerative disease that requires a compassionate and committed social worker who is comfortable with end of life care.

In collaboration with the NYC and LI care teams, this full-time, home-based position will conduct virtual and in-person home visits offering social work assessments, short-term supportive counseling, case management, and crisis intervention. This position will also require the co-facilitation of support groups for patients and caregivers, as well as coordination of monthly educational programs all of which, at present, are being conducted virtually. In addition, the position requires periodic participation in chapter-wide events, some of which take place on weekends and evenings.

QUALIFICATIONS:

· LSW with active NY State license

· At least three years experience in similar role

· Experience in end of life care

· Strong communication and interpersonal skills

· Strong team player

· Bi-lingual a plus

· High degree of initiative, able to work independently

· Experience in group work

· Valid driver’s license and use of car

Job Type: Full-time

Pay: $75,000.00 - $85,000.00 per year
