Signify Health is looking for nurse practitioners to perform pre-scheduled Health Risk Assessments (HRA) with Medicare Advantage members on health plans that we represent. The HRA’s consist of a history and physical evaluation that is performed in the member’s home. By capturing complete health and social welfare assessments of health plan members to drive better clinical outcomes through care management referrals and return-to-care recommendations, Signify Health’s insights help enhance member engagement, improve quality ratings and reduce overall healthcare costs. This is a travel position which does require covering up to a 2 hour radius as needed.

* $88-$128 PER ASSESSMENT COMPLETED (based on availability)

* Flexible schedule: Monday through Sunday, 8am-8pm

* Part time, full time, or PRN availability considered.

* In-home assessments

* Prescheduled H&P’s / HRA’s

* Hotel, Rental Car, and availability incentive provided for qualifying assignments

* Independent Contractor, 1099 position

* Nurse Practitioner license and board certification required. You will not be required to prescribe medicine, order lab tests, do blood work or alter the member's current treatment regimen. We will provide you with a pre-populated, patient-specific form for each evaluation with the demographic information and medical history of the patient as made available by the insurer.
