We are seeking Physician Assistants and Nurse Practitioners to join our team as Advanced Clinical Providers. As an Advanced Clinical Provider, you will focus on providing high-quality patient care as part of a healthcare team, right in patients' homes.
The successful candidate will be a multi-faceted Nurse Practitioner or Physician Assistant with a strong and demonstrated orientation to improving patient outcomes, a desire to practice within the full scope of his/her training and experience, and exhibit a high degree of professionalism. This candidate will operate as an integral part of a multi-disciplinary team within a growing company.
Responsibilities:
- Diagnose and treat acute, episodic, or chronic illnesses
- Prescribe necessary medications and treatments
- Perform comprehensive patient examinations
- Develop and implement patient management plans
- Facilitate referrals to other healthcare professionals and medical facilities
- Communicate with collaborating physicians, specialists, and communities regarding patient care
Qualifications:
- Deep commitment to providing complete care
- Ability to build rapport with patients
- Excellent written and verbal communication skills
- Strong leadership qualities
- Desire to engage in team-based healthcare
- Multi-lingual a plus, especially Spanish
- Experience in house-call / in-home medical care is a plus
Salary & Benefits:
- Experience-Based Starting Salary Range: $100,000 to $138,000

Area: Greater Atlanta Metropolitan Area, and surrounding areas.
Experience or certification in Wound Care, Diabetes Management, and/or Behavioral Health is a plus.
