URGENT Home Health Occupational Therapist / Physical Therapist Positions

We are looking for OT's and PT's who are passionate in providing quality care and:

• who are enthusiastic in making a difference in our patient's lives
• have excellent interpersonal skills
• have strong clinical decision making

QUALIFICATIONS:

• BCLS certification
• Active OTR/L or PT License in NV, with no active disciplinary restrictions
• 2 years of Home Health experience preferred

Responsibilities:

1. Improves or minimizes residual physical disabilities of the patient.

2. Returns the individual to optimum and productive living within the patient's capabilities.

3. Periodically participates with all other home health personnel in patient care planning.

4. Provides prescribed occupational therapy.

5. Responsible for initial assessment, plan of care, maintenance program development and modifications and reassessment every 30 days.

6. Performs all skilled procedure.

7. Consults with Providers and writes report regarding patient's progress.

8. Instructs patients/family members in home programs and fine motor movement exercises.

9. Assists the Provider in evaluating level of function.

10. Helps develop the plan of care and revises as necessary.

11. Prepares clinical and progress notes.

12. Participates in QAPI activities as assigned.
